The aim of this program is to provide a comprehensive range of services to people
in their own homes. Community Care Packages (CCP's) are targeted to those older people
living at home who have:
- Been assessed as eligible to receive a CCP by the Aged Care Assessment Service.
- Complex care needs including physical, social and emotional.
- A need for ongoing monitoring and reviewing of their changing care needs.
Community Care Packages are:
- Planned & coordinated packages of community care services to help people with complex
care needs remain living in their own home.
- Very flexible.
- Designed to help with individual care needs.
- Funded by the Commonwealth Government to ensure high quality care.
- Provided within a limited area of Brisbane South.

Services Offered:
- Assistance with showering, bathing, personal hygiene, dressing & grooming
- Laundry Assistance
- Shopping
- Meal Preparation
- In-Home respite for carers
- Housekeeping
- Transport to and from appointments (eg medical)
- Culturally appropriate assistance
- Services provided are very flexible and can change as needs change.
Who is eligible? Older people with complex care needs that require assistance to
enable them to continue living in their own community.
How to apply? To be eligible to receive a CCP, your needs must first be assessed
by the Aged Care Assessment Service. They can see people in their own home, in hospital,
or wherever they are currently residing and can be contacted by:
- yourself,
- your doctor,
- visiting nurse,
- social worker or other health professional,
- relatives or friends,
- other service.

What happens when I am approved for a CCP?
- The Flexi Care Inc CCP Coordinator will give you information to help you make informed
choices about the services you can receive.
- You will be offered a written agreement which clearly sets out your rights and responsibilities.
- Provide you with a written care plan of the services you will receive.
- Our Coordinator/Case Manager will regularly review your situation to make sure the
services you receive are appropriate, and monitor the delivery of those services.
- You may be referred to other service providers if Flexi Care Inc can no longer meet
your needs.
Are there any fees for these services?
- For older people on the maximum basic rate of pension, fees will not exceed 17.5%
of that pension.
- You will not be denied a service you need because you cannot afford to pay. Each
case will be assessed on its own merits.
- People on higher incomes may be asked to pay additional fees.
What are my rights if I receive a CCP?
- To be involved in deciding which care services are the most appropriate for your
needs.
- To have services that take account of your lifestyle, cultural and religious preferences.
- To be treated with dignity and have your privacy respected.
- You can complain about the services you receive without fear of losing services,
or being disadvantaged in any way.
- You can choose a person to speak on your behalf for any reason.
- To be able to take part in social and community activities as you wish.
